CUSTOMER STATED: They had repair made through choice warranty, and shortly after repair the tub/shower was leaking again. TECHNICIAN NOTES: This is a return visit, and I was authorized through choice to put plumbers putty and tighten overflow plate as it was loose for repairs. I let home owners know that before I left the site on last visit that this was not a permanent solution, and we would need to replace the whole trap, and overflow assembly. I let home owners know that I would submit to choice warranty, but there is a possibility this would not be covered as this is not normal wear and tear. RESOLUTION: We will need to replace the trip waste, overflow, and trap for the upstairs guest bathroom tub in order to help fix/prevent any future plumbing issues. JOB NOTES: STATUS: Complete AUTHO: Submitted to choice warranty ESTIMATES: Please FWD

CUSTOMER STATED: His Circuit breaker outlet for his garbage disposal was tripping, and was doing it even when he was not using it. TECHNICIAN NOTES: Upon inspection I found that this is a insinkerator badger 1 garbage disposal located in the main level kitchen. I was unable to get the disposal to trip at the breaker while on site. I turned disposal on and off multiple times, and even ran it for 5 mins straight without any issues from disposal or breaker. I made home owner aware if I can not recreate the issue while on site today choice warranty is not going to cover anything. I let home owner know this seems to be an electrical issue, and would contact an electrician to further diagnose. I also let him know if it would make him feel better we could replace disposal, but it would be through service not warranty as there are no mechanical failures while on site. RESOLUTION: There are no mechanical failures or plumbing issues while on site for todays visit. JOB NOTES: **Submitted

Customer Stated: Their toilet was rocking. They also stated previous plumber came out twice to make repair, but after he had made repair it was still rocking. Tech Notes: Upon inspection I found that the toilet is located in the main level master bathroom. I was able to find that the toilet was rocking significantly, and had rubber shims under it. I let home owners know that this would not be covered through AHS, and in order for me to further diagnose I would need to pull the toilet to inspect flange. Home owners elected to go through AHS. They are interested in the home care club membership. Resolution: We will need to pull the toilet to further diagnose water closet flange. Job Status: Completed Estimates: Please FWD Autho: 115ACLI

Customer Stated: She thought her toilet could possibly be leaking, and was hearing a dripping noise. Tech Notes: Upon inspection I found that this is a American Standard elongated toilet located in the upstairs master bathroom. The toilet seems to be a newer toilet, and everything is functioning properly when flushed. There are a slow couple of drips after flushing when fill valve is filling within the tank that create the dripping noise home owner is hearing, but then it stops. Everything upon inspection is in good condition, and functioning properly. There are no mechanical failures while on site for todays visit. Resolution: There are no mechanical failures while on site. If home owner would like we can change out the existing fill valve. There are no further plumbing issues for todays visit. Job Status: Complete Estimates: Please FWD Autho: 115RTAA

Out today to inspect the plumbing system prior to a 5/23 move in. When I arrived the water was on at the main and I found no supply leaks or drips. I did find multiple leaks on the kitchen drain lines that required total rework and rebuild, Leaks are fixed and all functioning properly. Additionally was asked to inspect septic system that was to have been recently pumped. I DID NOT FIND any signs of this septic system being serviced in any way. There was no excavation or disturbance of the yard that I was able to locate. Based on the lay out of fittings I was unable to run our camera. Job complete Please have Chris bill out Water pressure at both bibs is 180 PSI but I believe they are pre Prv.

Customer stated: they had a leaking shower and a leaking sink. Technician notes: called out today to look at a leaking shower and a leaking faucet. While on site today I found that the sink in the guest bathroom had no leak. I had the water running for 5 minutes and could get the leak to reshot itself. While looking at the shower I found that the shower arm had a drip to it. Resolution: I took the shower arm off and resealed it with Teflon and pipe dope. After reattaching the shower arm I found no leaks or drips. Autho: auth42664330 Job status complete.
