CUSTOMER STATED: Customers kitchen Drain line was backed up, First technician that American home shield sent out, did not properly diagnose the issue and left before starting the job. They had an emergency leak on Sunday which they had their own service contractor come out to replace the cracked cast iron. When other technicians sent his small machine down the pipe, it broke his equipment. TECHNICIAN NOTES: Technician found that the kitchen drain is draining, extremely slow due to the age of the cast iron and most likely kitchen grease that developed over the course of the 50 years The home was built. All the cast-iron about 30 to 40 feet looks like it was improperly installed with no proper incline which Is causing the water to backflow in the other direction. RESOLUTION/RECOMMENDATIONS: Due to the other technician breaking his equipment, we will not be sending down any of our equipment down that pipe due to risk of Breaking. CUSTOMER STATED: TECHNICIAN NOTES: not a plumbing issue, has will need to send out a tile contractor to fix the broken seal on the shower door, causing water to expect the shower completely which caused the leak in the cealing. Water heater is currently leaking due to age. Unit is a natural gas heater built in 2001, will need to completely replace the unit to resolve the issue with all required code items. Will most likely need a tall skinny unit due to the area we have to work with currently. Tech will be sitting with Dan to create a proper estimate.. Customers pressure regulating valve is leaking but the pressure is 55. Customer will need to call in a new work order for the replacement of the unit. 275rpra - autho

The homeowner complaint is that the water heater is not heating the water and there is a slow leak from the tank I found that there is a leak showing up on the water heater on the left And the pilot light is out The water heater is a 40 gallon natural gas It is on a stand in a utility closet The water heater need to be replaced The 2nd water heater is the same age and I’d recommend to replace both Job not completed Autho need to get with management to get authorization Estimates are started need to get with management

AHS return trip to replace parts provided by the manufacturer. Cartridge, handle, bonnet nut and plastic cover piece. Plastic cover piece will need broken off as it is not budging without damaging. Customer will call once parts arrive. Customer States: Parts arrived from manufacturer and needs them installed. Technician Notes: Out today to installed manufacturer supplied parts on the Pfister single handle kitchen faucet. Resolution: Installed new cartridge, handle, and trim. Restored water to faucet and tested for proper function and leaks. Customer will monitor over the next couple weeks to make sure issue is handled.

Custumer stated pop up assembly was broken. After inspecting I found the pop up rod needed to be replaced AHs denied the repair due to preexisting conditions. The home owner authorized the cod estimate to replace the rod only. After installing I tested and found no further leaks or drips at this time.

CUSTOMER STATED: Leak in kitchen ceiling TECHNICIAN NOTES: Drop ceiling in kitchen and you are able to remove tiles and the area of concerns is beneath the master bath shower stall RESOLUTION: Homeowner removed tiles and did let me know that the leak is very small and speratic. I ran water and was unable to recreate the leak. I did notice that the underside of the drain in the all tile shower is cracked but I did not see a leak from that area. I would refer this back the the home warranty company for a tile contractor to investigate. JOB NOTES: STATUS Complete no mechanical failure AUTHO 275 RQSA ESTIMATES None

CUSTOMER STATED: Out today to provide estimates for a leaking kitchen faucet TECHNICIAN NOTES: This is a single handle brushed nickel kitchen faucet with integrated sprayer. Faucet would leak when water was turned on to run. RESOLUTION: I was able to determine that the leak was coming from the body of the faucet and not the sprayer hose and would recommend replacement. AHS estimate to replace the 40 gallon natural gas water heater located in the basement/ crawl space. Includes code requirements and a permit. Unit is located in the back of a crawl space. Will have to be brought in under a low hanging HVAC unit. Measured space at 23” tall to fit water heater under. Existing unit is around 21” in diameter and sits 48 1/2” tall. There is roughly 72” from floor to ceiling.
