CUSTOMER STATED: water heater has failed it is leaking TECHNICIAN NOTES: came out to location for diagnosis found water heater leaking RESOLUTION: return trip for install of water heater, found no further issues with unit leaking installed a new 50 gallon electric water heater located in basement. Water had leaked prior to me arriving and I cleaned up as much as I could. New water heater is on I found no leaks or issues with unit MGPNOTES: JOBSTATUS: complete ESTIMATES: AUTHO: This includes expansion tanks, sweat shut off, dielectric unions, pan and permitting for the unit.

Customer States: Toilet is leaking. Turned water off to toilet. Technician Notes: Out today to inspect toilet in the upstairs master bathroom that is leaking. Found that one of the tank bolts dropped. Left side tank bolt was on the ground. Right side was about to do the same. Resolution: Removed old tank bolts and installed new left and right tank bolts in the upstairs master bathroom toilet. Tested for proper function. Job Status: Complete. CUSTOMER STATED : Water heater is not producing hot water TECHNICIAN NOTES : Out to the residence today for the concern of the existing water heater not producing hot water. When i arrived the homeowner brought me to the garage where I found a 50 gallon natural gas water heater. While trouble shooting water heater, I found the burner assembly was not able to hold a flame. Existing water heater is 18 years of age and I do recommend replacing due to the age and condition of the existing water heater. Choice home Warranty approved the burner assembly to be replaced. RESOLUTION : Need to reschedule when we have the correct A.O. Smith gas control valve so we can replace it JOB NOTES : Job not complete Please forward all estimates to homeowner No payment to collect from the homeowner at this point in time.

Customer Stated: Her toilet water would come up, and then go back down. She thought there was some sort of clog issue. She also stated that it would constantly run. Tech Notes: Upon inspection I found that this is a Gerber elongated toilet located in the basement of home. I flushed the toilet and found that the fill valve took a very long time to shut off, and when I shut off the compression valve it seemed to still be filling but would then shut off again. I let home owner know I could replace the fill valve through warranty but that this will not help with the flushing of the toilet. I let her know that we could also try doing a major rebuild, but this would also have no affect on the flush power for the toilet. I let her know that her best permanent solution would be to replace the toilet. I presented options on having the toilet, and compression valve replaced through service as since there are no mechanical failures while on site today this would be an out of pocket expense. Home

TECHNICIAN NOTES: I need to get with Chris for a re pipe estimate due to not finding any leaks today, main issue will possibly be the material used could’ve been faulty material or material with stress cracks, but no certain way to tell. This issue seems to be on going with repair and repair so reptile estimate will be built and sent over to AMH and findings will be sent over. I found no leaks today but from tenants description of leaks popping up it could be faulty material, I checked pressure and found it to be at 60 psi. 2 bathrooms 1 half 1 kitchen Washer box 2 right and left hose bib Maybe ice maker Fill fixtures 10 Half fixtures 3 2 half colds hose bibs 1 ice maker half 1 washer box full

CUSTOMER STATED: toilet is loose and rocking back and forth TECHNICIAN NOTES: came out to location for diagnosis of the toilet located in the master bathroom while checking the toilet I found that the toilet is moving side to side, the toilet was also rocking front to back as well. RESOLUTION: I tighten the bolts to a certain extent and added shims to the bottom of the toilet on the front side, I also added caulking and explain to the customer to let caulking dry for an hour or two, this resolve the issue of the rocking toilet as of today, and I found no further issues.
